Smartlipo represents another variation on liposuction technology. As long as you are a controlled diabetic, there are no increased risks for you to undergo any form of liposuction treatment. What may have attracted you to Smartlipo is that it is frequently marketed as being done under local anesthesia. That is a function of the comfort of the practitioner performing it, how tolerant the patient is, and how much fat you have to remove to get a good result. Smartlipo is a good liposuction advancement but it is not magic and is still a surgical procedure.

As long as your diabetes is under control, you are able to do Smartlipo. The main factors that are involved with the procedure include how much fat is present, how much loose skin (if any), and current collagen state. You are 38, and young, so I believe you would have some good collagen denaturing from the laser, and in turn see some nice skin tightening. The amount of fat that needs to be taken out is determined during consultation. Usually, a physician will use another lipo technology to remove the fat (vaser, vibro lipo, pal) and then come back over the same area with the smartlipo laser to melt any remaining fat, and tighten the skin.
