There is often a tough dead skin cell layer in the regions you mention. Microdermabrasion, and the application of topical products containin retin A and 4% hydroquinone (example- Obagi Clear cream) are helpful. The sides of the face will respond to these as well, and also to an IPL photofacial.

I would suggest using an over the counter glycolic acid cream or lotion for several months. Only use it once a day.

In situations where the pigment has been caused by overuse of topical skincare products, we recommend either stopping the use of all products for a period of time to let the pigment fade or consider the use of laser treatments to remove the pigment. We are not sure about the
pigment on the hands, but if the facial pigment is Melasma we would not treat it with lasers, but with topicals.
