Should I do Radiesse again, do I need multiple treatments?

I had one injection of Radiesse 11/13, looked great by 11/15 and is now 85% gone from my face. I still have a bruise on my chin area. I am 62 years old. Will another injection of Radiesse make a difference and will I bruise again?

My guess is that since you are not seeing a difference, not enough filler was used. Many people need 2 or 3 syringes to get a really nice result. Understand that a syringe of filler, example Radiesse, is only 1.5 ml of volume. I have used 3 or 4 syringes at times in someone who is in their 60's or 70's....

Unfortunately, another treatment will probably cause more bruising. It is not physically possible that the Radiesse is 85% gone already; you must have had a lot of swelling after your treatment giving the impression that it caused more filling. Adding more filler will give you more results although I don't know exactly what is going on with your face.

The best results with filler are those that are sculpted over time. The client and injector should consider the face like a canvas that is shaped and molded to create the best long-lasting result. It is best to allow the swelling to resolve, reassess the individial reaction and come back later to continue the process and add adjustments, just like you would a piece of sculpture. You do not have one shot to get everything the way you want.
Radiesse is a calcium-containing product in a water based gel. The water and swelling that are partially enhancing the immediate results dissipate and you are left with the actual implant itself. Radiesse stimulates collagen growth, which may later enhance and prolong your result. You should also consider whether you were treated to full correction. If not, optimal results are compromised. Bruising is determined by the amount of blood vessels in the area, (some areas are more vascular than others), your age, number of injection sites and integrity of your blood vessels and skin. It may be hard to predict but the areas around the mouth are susceptible.

I think it is a little to early to decide. Radiesse has a gel carrier that is reabsorbed over time. It would be very unusual for the gel carrier to go away in just a few days. I would assume that the injectionist added some lidocaine to the Radiesse to help with pain
control. The lidocaine is reabsorbed very rapidly and you likely had some swelling that has gone down as well. I would wait another month or so then decide.

Radiesse does not need to be replaced for 12 - 13 months and certainly not after only 1-2 weeks. Most likely, you had some swelling from the injection, which can take up to 2 weeks to go down. I tell my patients to give it two weeks then look at the results. Often, I will have them come back for an appointment, especially if it is their first treatment. It
sounds like you may not have gotten enough Radiesse initially and now that the swelling has gone down, it looks like it is "gone," but it really isn't gone, there just wasn't enough of it to begin with. I think that is OK.

You can always add more, but you can't take it away if you get too much. So whoever injected you did the right thing - I always err on the side of conservatism. In other words, I would rather give someone too little, than too much. So, if you think you need more; go get more! Oh, and you may bruise again. Make sure you are not taking anything which increases your risk of bruising: Aspirin, ibuprofen, naproxen, or other non-steroidal
anti-inflammatory, fish-oil, vitamin E, plavix, coumadin, lovanox, to name
the more common.

Some swelling occurs after any injectable product is given. When the swelling goes down, the patient gets to see the true result of the product. You probably need more Radiesse, if you liked it better with the swelling present. Patients and physicians struggle with two opposing goals when doing dermal fillers. One the one hand, you'd like to have one session so as to go through the pain, swelling and bruising one time; on the other hand, we can always put in more product, but we can't take it out. So each session is a balance of trying to put in just the right amount of product to get the desired result without putting in too much. As we are generally conservative by nature, a second session is not uncommon.

The Radiesse cannot be gone in a couple of days but may have migrated to an area where it is difficult to see. I suggest talking to your injector and getting their thoughts.

That's a very good question and there are a few answers. Many patients believe they know what they need and come in for a syringe of Restylane, Juvederm or Radiesse believing one syringe will make a difference when they really needed 3-5 syringes. Some patients do only require one syringe however the area of injection may have swelled a bit and caused the area
injected to look fuller then it actually was. In that case it's better to stop at 1 syringe, wait a few days and then inject additional syringes. It always easier to add additional syringes then have to remove excess product especially in the case of Radiesse.
