There are a lot of facial fillers now on the market. The bulk of them are made of hylauronic acid which is naturally occuring in our bodies. This makes those products safe with little or no allergic reactions. Juvederm and Restylane can last up to six months. Perlane can last up to 9 months and Radiesse made from a calcium based product has been advertised to last around one year. Evolence is promoted to last through 12 months. I have seen these products last longer than advertised and I have seen them last half the time. It really depends upon the person and how fast their body breaks down the product. Which ever you end up going with, they all last longer than the old fashioned collagen versions that were made from cows or cadavers.

While there are now nearly a dozen available injectable fillers, the practical differences between them come down to how long they last and how much they cost for the traditional uses of nasolabial reduction and lip augmentation. The differences in them chemically and in their brand names does not change what they are all approved for...soft tissue augmentation. Many doctors favor one over another and they may have very valid reasons, but you should ask why.

The temporary fillers include Collagen from a bovine source {Zyderm, Zyplast], from a human source [Cosmoderm, Cosmoplast], from a porcine source [Evolence], and Hyaluronic Acid [Restylane, Prevelle Silk, Juvederm]. Which is "best" depends upon the location and experience of the injector. The results are highly dependent upon the skill and experience of the injector. The injector can explain why they like a particular product for a particular problem. Some of these products may be part of studies which show them to last longer than other ones. However, I would think of them as a similar group of products. Semi-permanent fillers include Radiesse, which is calcium hydroxyapppatite, and Artefill, which is polymethylmethacrylate. These products may last for several years, so they can be more cost effective. However, this means that if there is any problem it will also be there for a long time. These products may be more appropriate in areas with thicker skin coverage like the nasolabial crease. It may be safer to have a shorter acting filler first to see if the effect is what you want before trying the longer acting filler. Some newer fillers like Sculptra, which is poly-L-lactate, act by stimulating the body to form collagen. Sculptra is used deeper to provide bulk rather than for fine lines. There are many other products available in other countries and new ones are regularly approved in the United States, so the options available are constantly changing.

There is no "best" dermal filler. All of the dermal fillers have different properties in terms of ease of use, longevity, smoothness, risk of allergies and other characteristics that need to be considered before a filler is chosen for a particular patient. It is important that a provider evaluate the goals of the patient, their risk tolerance and the physical needs to determine which dermal filler would be best. In many cases, different fillers may be used on the same patient for different areas. Patients should ask their physician why a particular filler is recommended for them and inquire about the risks and benefits of its use. With the numerous fillers on the market today, providers have a variety of choices that enable them to give the desired results while limiting the risks. Together the patient and the physician can agree on a filler that is best for them.

Currently, Radiesse has FDA approval and clinical data showing results up to 13 months from the time of injection. Evolence is approved for 12 months; Juvaderm for 9, although, in practice, we tend to actually get closer to 12. So, they are all very close in terms of duration, but Radiesse seems to last the longest for now.

I personally like Juvederm and Evolence the best. We know Juvederm lasts around a year. Evolence has gotten the approval for 1 year but I have only been using it about 8 months. Patients seem to like it a lot. One of it's benefits is that it does not bruise much.It flows very nicely and has minimal bruising. Sometimes you need 2 syringes to get the same filling as Juvederm. Juvederm can definitely bruise. For the most long term filler, I like Artefill. I would prefer to use this on a patient that has had fillers in the past and knows that they like the look. Artefill lasts years. It also requires a 1 month skin test.

There are many new fillers in the market. The choice depends on what you are looking to correct. Some of the popular fillers are: Restylane Perlane Juvaderm Ultra Juvaderm Ultra Plus Radiesse Evolence, recently introduced fillers made of hyaluronic acid (HA), which is a substance naturally found in our body. The difference between these fillers are the manufacturers and their manufacturing process. They can last any where from six months to a year. At our practice we mainly use the HA fillers and achieve great results. We like Juvaderm Ultra for the lips. facial fold and wrinkles. We like Radiesse for volumizing the hands. The results can last up to a year or more depending the client and how his/her body metabolizes the product. collagen, a protein that is responsible for skin structure and strength. It does not require any skin testing. Unlike other dermal fillers, Evolence causes little or no swelling, or bruising, or even redness. It can last six to 12 months.

The choice of 'best' filler depends on the amount of volume loss and where it is going. Radiesse lasts in tissues the longest (well over a year) and stimulates new collagen production- however, it cannot be used in the lips, for examples, where restylane or juvederm ultra are preferable. Among the softer (hyaluronic acid)fillers, Juvederm Ultra or Ultra Plus in my experience last the longest. Any filler can be used in the nasolabial folds (side of nose to corner of mouth) or marionette region (corner of mouth to jawline). Sometimes we use a combination- such as a syringe of Radiesse plus one of Juvederm Ultra. Most important also is the technique/experience of the injectortoo so be sure that the person you consult with is very experienced and shows you before and after photos of their own clients (not stock photos from the company that makes the filler!!) Hope this helps!
